Family Medicine Physician Requirements (MD/DO)

• Education: MD or DO
• Board Status: Board Certified required; Board Eligible considered if within 5 years of residency
• Licensure: Active, unrestricted California medical license
• DEA: Active DEA required prior to start
• Certifications: BLS
• Credentialing/Compliance: Ability to meet immunization and screening requirements (e.g., seasonal flu, TB status, MMR/Hep B/Varicella immunity, Tdap within 10 years)
• Application Notes: CV must include month/year for work and education history with all employment gaps accounted for

Outpatient Primary Care Responsibilities

• Provide comprehensive outpatient primary care across the lifespan (adult and pediatric mix)
• Maintain the expected patient schedule of 18 patient encounters per shift (18 PPD; minimum see 18 PPD)
• Evaluate, diagnose, and manage acute and chronic conditions, including some chronic pain management
• Deliver preventive care and health maintenance (screenings, counseling, immunization guidance)
• Perform and/or coordinate common Family Medicine services; well-woman care and IUD insertion/removal are preferred (not required)
• Document care accurately in the medical record and close charts within 72 hours
