Join a value-based, team-oriented primary care model focused on quality over volume. This full-time role offers smaller patient panels, robust clinical support, minimal call, and strong compensation—allowing you to spend more time delivering meaningful, patient-centered care.
Job Responsibilities
• Evaluate, diagnose, and treat adult and senior patients
• Coordinate care including specialty referrals, hospital & SNF transitions
• Collaborate with multidisciplinary care teams through daily huddles
• Complete timely and accurate EMR documentation with coding support
• Participate in quality improvement and outcome review initiatives
• Deliver high-quality, patient-focused care in a value-based model
Qualifications
• MD or DO from an accredited medical school
• Board Certified / Board Eligible (FM, IM, or Geriatrics preferred)
• Active or eligible state medical license
• Active DEA license
• Medicare & Medicaid provider enrollment
• Strong communication and team-based care mindset
Working Hours
• Full-time
