👥 Collaborative Care Team

You’ll be supported by an integrated team including:

• Advanced Practice Providers

• Pharmacists

• Care Coordinators

• Behavioral Health Specialists

• Coders & Referral Coordinators

🩺 Key Responsibilities

• Evaluate and treat patients per established standards of care

• Coordinate specialty referrals and transitions (hospital/SNF, DME, home health)

• Participate in daily team huddles

• Partner with leadership to maintain a positive, cooperative environment

• Complete timely and accurate documentation with coder support

• Follow clinical policies and protocols

• Review quality outcomes and medical records with leadership

✅ Required Qualifications

• MD or DO from an accredited program

• Board Certified or Board Eligible (ABMS/AOA) in:

  • Family Medicine

  • Internal Medicine

  • Geriatrics

• Active and unrestricted state medical license (or ability to obtain)

• Active and unrestricted DEA
  

⭐ Preferred Experience

• Knowledge of HEDIS/STARS metrics

• Familiarity with CAHPS and HOS

• Experience in underserved or culturally diverse communities

• Experience with EHR systems

• Background in Medicare Advantage Global Risk models
