- Other details: The facility which is stroke-certified is looking for a provider to cover on-site clinical hours only. There are an average of 18 patients per shift and 1 bed in the department. Candidates must include the month and date on their work experience and education, with all employment gaps accounted for. The role involves chronic pain management chart closures within 72 hours, with a schedule of 18 patients per day (PPD) Monday through Friday, 8 am to 5 pm, with the possibility of 10-hour shifts. The patient mix includes both pediatric and adult populations, and candidates with a family medicine background will be considered. A decision is typically made within 7 to 14 days. Both MD and DO candidates will be considered, with Board-Eligible candidates accepted if they are within 5 years post-residency. An active DEA is required before the start date, and credentialing takes 30 to 60 days, with six weeks being common if the provider submits their application and supporting documents promptly. Required immunizations before the start date include the 2025-2026 flu vaccine, TB status/X-ray results (within 90 days), positive MMR titer or complete immunization series, positive Hep B titer or complete immunization series, positive varicella zoster titer or complete immunization series, and Tdap immunization (within the last 10 years).

Job Responsibilities

Qualifications
* Active DEA required before start date
* At least 5 years of relevant experience in chronic pain management (preferred)
* Immunization requirements: 2025-2026 flu vaccine, TB status/X-ray (within 90 days), positive MMR, Hep B, Varicella Zoster titers or complete immunization series, and Tdap (within 10 years)
* No gaps in employment, or all gaps accounted for with a clear explanation
Working Hours
* Minimum 18 PPD, with a mix of pediatric and adult patients
* Decision typically made within 7-14 days of application
Skills

* Strong organizational skills for charting and documentation
