We are seeking a Board-Certified Primary Care Physician to join a collaborative, mission-driven outpatient practice in San Jose, CA. ๐Ÿ‘ฉโ€โš•๏ธ Role Overview

You will provide comprehensive outpatient medical care to adult and pediatric patients, emphasizing preventive medicine, chronic disease management, behavioral health integration, and patient education.

๐Ÿ’ผ Key Responsibilities

  • Provide comprehensive primary and preventive care to adult and pediatric patients

  • Perform physical exams, diagnose conditions, and develop treatment plans

  • Order and interpret diagnostic tests and labs

  • Prescribe medications and deliver patient education

  • Conduct annual wellness visits and participate in group visits as appropriate

  • Support patient self-management and behavior modification strategies

  • Coordinate referrals and collaborate with subspecialists

  • Work closely with multidisciplinary clinical and behavioral health teams

  • Supervise and mentor APPs, residents, and medical students (as applicable)

  • Maintain accurate, timely EHR documentation

  • Participate in quality improvement initiatives, peer review, and audits

  • Participate in rotating after-hours call coverage

  • Provide cross-coverage for colleagues as needed

  • Engage in community outreach initiatives

โœ… Required Qualifications

  • MD or DO degree

  • Board Certified in Family Medicine, Internal Medicine, or Pediatrics

  • Active, unrestricted California medical license

  • Active, unrestricted DEA license

  • Strong communication and interpersonal skills

  • Ability to thrive in a multidisciplinary, team-based care model

  • Experience in community health or integrated care settings

  • Knowledge of state and federal healthcare regulations

  • Bilingual proficiency preferred (Mandarin, Cantonese, Spanish, or Vietnamese)
