🩺 Key Responsibilities

• Provide primary care services including preventative, acute, and chronic disease management

• Conduct patient assessments, develop treatment plans, and manage follow-up care

• Prescribe medications and provide patient counseling on health management

• Maintain accurate and timely documentation of patient encounters

✅ Required Qualifications

• MD or DO degree (or equivalent)

• Current and unrestricted Illinois State Medical License

• Active DEA Certificate

• Board Certified or Board Eligible (actively pursuing certification preferred)

• Ability to successfully complete credentialing and obtain full clinical privileges
