Job Responsibilities
• Provide comprehensive primary care for patients of all ages
• Diagnose and manage acute and chronic conditions
• Perform preventive care, wellness exams, and screenings
• Develop individualized treatment plans
• Prescribe medications and order/interprete diagnostic tests
• Maintain accurate EMR documentation
• Collaborate with interdisciplinary care teams
Qualifications
• MD or DO degree
• Board Certified/Board Eligible in Family Medicine
• Active Illinois medical license (or eligible)
• Strong commitment to patient-centered care
Working Hours
• Full-time schedule
• Monday–Friday (Outpatient setting)
