Family Medicine Physician – Outpatient, Primary Care
A reputable, growing ambulatory health network is seeking a dedicated Family Medicine Physician to join its growing outpatient team. This opportunity is ideal for clinicians who are passionate about comprehensive primary care and committed to serving diverse patient populations within a community-focused setting.
This is a full-time, clinic-based role providing outpatient-only care with no hospital rounding and no after-hours call responsibilities. Physicians are supported by a robust administrative and clinical care team and enjoy the autonomy to practice high-quality, relationship-based primary care.

- Outpatient Only – no inpatient rounding
- Streamlined, physician-friendly referral network
- Strong clinical support and autonomy in clinical decision-making
- Easy access to specialty services within the network
- Ability to participate in teaching, QI, leadership, or community health programs
- A mission-driven team focused on population health and serving the community
- Provide comprehensive primary care to patients across the lifespan
- Conduct annual physicals, preventive visits, chronic disease management, and acute care visits
- Coordinate care with specialists, behavioral health, and ancillary services
- Engage in population health initiatives and community-centered programs
- Practice in a primary location with occasional coverage at nearby network sites
- Community-minded, mission-driven, and passionate about population health
- Collaborative, working closely with multidisciplinary teams
- Adaptable, with interest in teaching, program development, or administrative engagement
- Autonomous, yet eager to contribute to a team-based, supportive culture
- Leadership roles and medical directorship tracks
- Quality improvement initiatives

Schedule & Workload
- Monday–Friday, Days
- 1 Saturday shift per month (with a weekday off that week)
- 2 evening shifts per month
- No after-hours call
- No inpatient responsibilities
- MD or DO
- Board Certified or Board Eligible in Family Medicine
- Active NY medical license (IMLC accepted)
- ACLS certification
- Strong interest in outpatient, community-based primary care
