A well-established community health center is seeking a Family Medicine Physician to join its mission-driven team in Lancaster, California. This is a full-time, permanent opportunity focused on providing comprehensive, patient-centered primary care to a diverse and underserved population.

🩺 Key Responsibilities

As a Family Physician, you will:

• Provide comprehensive outpatient primary care for patients of all ages

• Diagnose and manage acute and chronic medical conditions

• Perform preventive care visits, annual physicals, and wellness exams

• Manage conditions such as diabetes, hypertension, asthma, and other chronic diseases

• Deliver women’s health services and minor in-office procedures (as applicable)

• Coordinate care with specialists, behavioral health, and support services

• Educate patients and families on disease prevention and health maintenance

• Maintain accurate and timely EMR documentation

• Support quality improvement and community health initiatives

✅ Qualifications

• MD or DO from an accredited medical school

• Completion of accredited Family Medicine residency

• Board Certified or Board Eligible in Family Medicine

• Active California medical license (or ability to obtain)

• Commitment to community-based, patient-focused care
