Seeking a Physician to join a post-acute care program in Pueblo, Colorado. This part-time opportunity offers a flexible schedule with a focus on inpatient rounding across post-acute care settings and collaboration with interdisciplinary care teams.
Position Highlights:
Part-time position, three days per week
Average patient volume of 15–20 patients per day
Customizable schedule to support work-life balance
No primary night call

Inpatient rounding in skilled nursing facilities, inpatient rehabilitation facilities, and long-term care hospitals
Collaboration with interdisciplinary clinical teams
Work closely with hospice, palliative care, and home care agencies
Well-developed infrastructure with extensive back-office support
EMR training and support using Gehrimed
40-day onboarding program with MIPS training and support
Growth opportunities into local and national leadership roles
Responsibilities:
Provide high-quality, safe, and cost-effective patient care in post-acute settings
Conduct inpatient rounding and manage patient care plans
Collaborate with interdisciplinary teams and follow CMS guidelines
Ensure safe patient transitions of care
Complete timely documentation and billing in compliance with Medicare requirements
Qualifications:
MD or DO degree
Board Certified in Family Medicine, Internal Medicine, or Emergency Medicine
Willing to train experienced candidates
Active Colorado medical license or eligibility
Compensation & Benefits:
Fee-for-service compensation model
Estimated annual compensation range of $248,000–$276,000
Uncapped productivity income potential
Part-time position not eligible for full benefits
Part-time benefits include license reimbursement
