Job Responsibilities
🕒 Manage comprehensive visits with a focus on prevention, chronic disease management
📋 Diagnose, treat, and monitor a range of medical conditions w/ continuity of care
🤝 Collaborate closely with a strong interdisciplinary team
🧠 Deliver thoughtful clinical decision making with time to educate patients and build trust
💊 Prescribe and manage medications while maintaining evidence based treatment plans
📈 Maintain accurate documentation and support initiatives that improve patient outcomes
❤️ Support a mission driven model of care focused on access, community health, and meaningful impact
Qualifications
🏆 Board Certified or Board Eligible in Family Medicine or Internal Medicine
📍 Active Oregon medical license preferred or willing to obtain
👩⚕️ Strong experience in outpatient Primary Care
Working Hours
📆 Mon thru Fri - 4.5 days per week
