Job Responsibilities
๐ Manage comprehensive visits with a focus on prevention, chronic disease management
๐ Diagnose, treat, and monitor a range of medical conditions w/ continuity of care
๐ค Collaborate closely with a strong interdisciplinary team
๐ง Deliver thoughtful clinical decision making with time to educate patients and build trust
๐ Prescribe and manage medications while maintaining evidence based treatment plans
๐ Maintain accurate documentation and support initiatives that improve patient outcomes

Qualifications
๐ Board Certified or Board Eligible in Family Medicine or Internal Medicine
๐ Active Oregon medical license preferred or willing to obtain
๐ฉโโ๏ธ Strong experience in outpatient Primary Care
Working Hours
๐ Mon thru Fri - 4.5 days per week
