- Provider Credentials: MD or DO
- Board Status: Board Certified or Board Eligible in Family Medicine
- Certifications Needed: Current and valid medical license in the state of Indiana
Position Details:

- Estimated Start Date: As soon as credentialed
- Estimated Privileging Timeline: Based on credentialing process
