Job Responsibilities
-Represent Bloom Healthcare in the San Antonio Market as a Bloom ambassador supporting our new market growth including market analysis to understand the local healthcare landscape and competitors.
-Strategic planning to grow your patient panel and deliver comprehensive primary care at home visits to 10 patients per day.
-Build trusted relationships by engaging with key members of our patients’ care teams, including assisted living facilities, hospitals, clinics, and community organizations, to facilitate coordination of care.
-Collaborate with interdisciplinary teams, including specialists, pharmacists, and social workers, to coordinate care, address complex patient needs, and achieve optimal health outcomes.
-Provide data-driven, condition-specific specialty visits to patients as needed
-Manage the health outcomes of an empaneled patient population
-Engage with and appropriately utilize population health data to drive superior patient outcomes
-Attend ongoing educational opportunities
-Commute to assigned assisted living communities to provide care at home
-Occasional travel to Denver market for initial and ongoing training
