As a physician, you will:
• Partner with local communities and senior living organizations to build our presence
• Help shape market strategy and patient panel development
• Influence clinical workflows and care practices as the market grows
• Build trusted relationships within care ecosystems across St. Louis
• Serve as a key ambassador for Bloom in one of our newest territories
Job Responsibilities
• Deliver comprehensive, relationship-based primary care in patients’ homes and senior communities tailored for medically complex cases.
• Manage your own empaneled patient panel with autonomy and support, focusing on quality, outcomes, and trusted care continuity.
• Provide condition-specific visits, chronic disease management, and individualized care planning.
Market Growth & Strategic Influence
• Serve as a founding physician leader in the St. Louis market helping to define clinical care pathways, community partnerships, and growth strategies.
• Develop relationships with assisted living partners, hospitals, clinics, and community organizations to expand Bloom’s footprint.
• Be a visible ambassador for Bloom’s model of care; one that is transforming healthcare delivery for underserved populations.
Team Collaboration & Clinical Excellence
• Work alongside interdisciplinary care teams including specialists, pharmacists, social workers, and care managers to deliver well-coordinated care.
• Use population health data to drive superior patient outcomes and continuous improvement.
Qualifications
• Board Certification or eligibility in Family Medicine, Internal Medicine, Geriatrics, or related primary care specialty
• Passion for relationship centered care and practice innovation
