This Family Medicine Physician opportunity is based in Woodland, California, at a community-focused outpatient health center serving a diverse and underserved population. Job Responsibilities
Manage preventive care, chronic disease, and acute medical concerns
Review and interpret laboratory results, diagnostic imaging, and clinical data
Prescribe medications and treatment plans while ensuring safe and appropriate use
Coordinate specialty referrals and collaborate with interdisciplinary care teams
Educate patients on preventive health, disease management, and wellness strategies
Perform in-office procedures within scope of practice and clinical competency
Triage patient needs and respond to clinical inquiries via phone and electronic messaging
Maintain accurate, timely documentation in the electronic health record
Work closely with medical assistants, nursing staff, and support teams to ensure continuity of care
Participate in quality improvement initiatives and clinical performance activities
Support medical learners and trainees as applicable
Participate in rotating after-hours outpatient call coverage
Maintain compliance with clinical guidelines, organizational policies, and professional standards
Qualifications
Active or eligible California medical license
DEA registration (active or ability to obtain)
Medication furnishing license (active or ability to obtain)
Current BLS certification (ACLS acceptable)
Prior experience in outpatient primary care; community-based care experience preferred
