Job Description:

Job Function:
- The organization provides full service primary care, dental, and mental health services to the communities of need.
- Providers provide routine primary care, wellness assessments, and references for specialty care.
- Provide patient care to patients of all ages.
- Evaluate patients medication needs, write prescriptions.
- Provide patient education around health promotion, disease prevention, and health maintenance.
Job Requirements:
- NY State license, DEA, CSR (active or applied preferred).
- Residency Training as Family Medicine (MD) required.
- Board Certification in Family Medicine required.
- +1 years of experience required.
