A mission-driven Federally Qualified Health Center (FQHC) in Connecticut is seeking a Family Nurse Practitioner (APRN) to provide comprehensive primary care to patients across the lifespan, from pediatrics through geriatrics. This role is ideal for a clinically strong, team-oriented provider who is passionate about delivering high-quality, accessible care in a collaborative outpatient setting.
The Nurse Practitioner will deliver direct patient care while working closely with physicians, nurses, medical assistants, case managers, and support staff to ensure coordinated, patient-centered care. This position follows all applicable Connecticut state practice requirements, including physician collaboration for newer APRNs.
Job Responsibilities
- Provide comprehensive primary care services within the scope of APRN licensure
- Conduct patient assessments, including medical histories, physical exams, and diagnostic evaluations
- Diagnose and treat acute and chronic conditions across the lifespan
- Order, interpret, and follow up on laboratory and diagnostic test results
- Collaborate with physicians and the interdisciplinary care team to develop and implement patient care plans
- Refer patients to specialists and community resources as clinically appropriate
- Participate in quality improvement initiatives, professional practice meetings, and staff meetings
- Engage in community-based services such as outreach events and health fairs as needed
- Maintain accurate, timely documentation in the medical record
- Support and advocate for community health programs and population health initiatives
Qualifications
- Active APRN licensure in the State of Connecticut (or ability to obtain)
- Family Nurse Practitioner certification required
- Eligible to practice collaborative care per Connecticut state regulations
- Experience in primary care or community health preferred
- Strong clinical assessment, diagnostic, and patient management skills
- Ability to work effectively in a team-based, mission-driven environment
- Commitment to professional development and continuing education
