Job description
We are seeking a Family Medicine Physician to join our innovative care team in Decatur, GA. Deliver whole-person care in 30- to 60-minute appointments, supported by a health coach and care team, focusing on high engagement, managed care, and culture transformation to reduce costly service utilization and improve overall health outcomes.
Position Highlights: See 10-15 patients daily in longer, meaningful appointments. Collaborative, team-based approach with health coaching support. Focus on proactive, value-based care
Requirements:
- MD or DO degree with completed Family Medicine
- Board Certified
- At least 3 years of primary care experience
- Current, valid medical license and DEA number
- Proficiency with EMR systems (NextGen preferred), coding, and Medicare Advantage/HCC documentation
- Experience with Google Workspace and team-based care
