Family Medicine Physician – Sonoma County Clinics
We are seeking a dedicated Family Medicine Physician to join our team across four community clinics in Sonoma County. This full-time position offers an excellent opportunity to provide comprehensive, patient-centered care in a diverse and collaborative environment.
Key Responsibilities:
- Provide primary care services to a diverse patient population, including preventive care, health education, diagnosis, and treatment of acute and chronic illnesses.
- Conduct thorough patient assessments, develop individualized care plans, and coordinate ongoing management.
- Collaborate with clinic staff, specialists, and other healthcare providers to ensure integrated and quality patient care.
- Maintain accurate and detailed medical records in compliance with regulatory standards.
- Participate in quality improvement initiatives and clinical protocols to enhance patient outcomes.
- Engage in patient education to promote health and wellness.
Qualifications:
- Medical degree (MD or DO) from an accredited institution.
- Board certification or eligibility in Family Medicine.
- Valid medical license to practice in California.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team-based environment.
- Prior experience in outpatient primary care setting preferred.
