Family Medicine Physician – Sonoma County Clinics

We are seeking a dedicated Family Medicine Physician to join our team across four community clinics in Sonoma County. This full-time position offers an excellent opportunity to provide comprehensive, patient-centered care in a diverse and collaborative environment.

Key Responsibilities:
  • Provide primary care services to a diverse patient population, including preventive care, health education, diagnosis, and treatment of acute and chronic illnesses.
  • Conduct thorough patient assessments, develop individualized care plans, and coordinate ongoing management.
  • Collaborate with clinic staff, specialists, and other healthcare providers to ensure integrated and quality patient care.
  • Maintain accurate and detailed medical records in compliance with regulatory standards.
  • Participate in quality improvement initiatives and clinical protocols to enhance patient outcomes.
  • Engage in patient education to promote health and wellness.

Qualifications:
  • Medical degree (MD or DO) from an accredited institution.
  • Board certification or eligibility in Family Medicine.
  • Valid medical license to practice in California.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team-based environment.
  • Prior experience in outpatient primary care setting preferred.

Position Details:
  • Employment Type: Full-Time (0.8 to 1.0 FTE)
  • Locations: Four clinics within Sonoma County
