Job Overview
We are seeking a skilled and mission-driven Family Medicine or Internal Medicine Physician to provide compassionate, comprehensive care for older adults in a community health center environment. This role includes outpatient primary care for frail and elderly patients, delivered within a collaborative, team-based model aligned with quality, service, and cost-effective care.
As part of a multidisciplinary care team, you will play an integral role in supporting patient well-being, promoting independence, and ensuring continuity of care for a complex population. We are a comprehensive healthcare program serving seniors 55 and older who require a nursing-home level of care but wish to remain independent in their homes and communities. Our integrated care model includes medical services, social support, rehabilitation, nutritional counseling, transportation, and individualized care planning.
With locations throughout Eureka and surrounding communities, our team is dedicated to delivering compassionate, coordinated care that enhances quality of life. Licensed by both the California Department of Health Care Services and CMS, we take pride in supporting older adults with dignity, respect, and a genuine commitment to their well-being.
Primary Care Physician
Salary Range: $218,192 – $284,689 annually + benefits
Requirements to Apply
- Current California Physician & Surgeon License with no past or pending disciplinary actions
- Current and valid DEA registration
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Minimum two (2) years of experience working with a frail or elderly population, ideally in a hospital or long-term care setting
- Strong computer proficiency and effective communication skills
- Self-motivated, flexible, and collaborative team player
- Bilingual English/Spanish preferred
