This is a full-time role that blends medical practice with compassionate advocacy, ideal for providers who want to focus on long-term relationships, preventive care, and improved quality of life—without the pressure of productivity quotas or fee-for-service billing.
You’ll work alongside an interdisciplinary team (IDT) of nurses, therapists, social workers, and specialists to co-manage care plans that emphasize dignity, independence, and continuity across settings. Daily schedules are balanced, with 5–7 visits/day, and you’ll support participants across adult day health centers, homes, and facility settings.
