A mission-driven community health organization dedicated to serving individuals with developmental disabilities is seeking a Primary Care Provider in Nassau County. Job Description:


Compensation : $200000 - $220000 Job ID : 35760

Job Function

  • Telephone-only call coverage, shared every 3 weeks; typically limited to 1–2 calls per shift, with most issues handled by the internal nurse triage team.

  • Expect to see approximately 15 patients per day, primarily adults; those with Family Medicine training may see pediatric patients as well.

  • Patient population is 90–95% individuals with intellectual and developmental disabilities (IDD); close collaboration with patients, aides, and family members is expected.

  • Electronic health records managed via eClinicalWorks (eCW).

  • Facility operates as a Patient-Centered Medical Home (PCMH), offering integrated services including primary care, podiatry, dental, psychology, and psychiatry.


Job Requirements:

  • Residency training in Internal Medicine or Family Medicine.

  • Active or pending New York medical license.

  • Board Certification or Board Eligibility required.
