Physician – Integrated Primary Care (Onsite)
Relocation assistance to Peoria, AZ available if needed

We’re looking for a dedicated, patient-centered physician to join a growing clinical team committed to delivering exceptional care in a supportive, team-based environment. This role offers the opportunity to practice high-quality medicine with the time, technology, and collaboration you need to make a meaningful impact on patient outcomes.

What You’ll Do:
As a key clinical team member, you’ll provide direct patient care while contributing to innovative quality and population-health initiatives. You’ll help shape a patient experience grounded in trust, accessibility, and clinical excellence. Responsibilities include:

  • Evaluating and treating patients in alignment with established clinical standards.

  • Monitoring care quality through data review, chart audits, and adherence to care pathways.

  • Coordinating services across the care team and referring to additional resources when appropriate.

  • Participating in daily huddles and serving as a medical resource for the team.

  • Maintaining a professional, approachable, and collaborative practice culture.

  • Completing timely and accurate medical documentation.

  • Upholding organizational policies, procedures, and quality expectations.

  • Collaborating with other departments on patient outcomes, process improvements, and clinical operations.

  • Identifying opportunities to expand or enhance clinical services.

  • Ensuring compliance with all licensing, credentialing, and regulatory requirements.

  • Building positive relationships with community partners and healthcare collaborators.

  • Working 100% onsite, with occasional movement between nearby clinics as needed.

  • Performing additional duties as assigned.

What We’re Looking For:

  • Current and unrestricted Arizona medical license.

  • Graduation from an accredited MD or DO program.

  • Excellent communication and interpersonal skills.

  • Strong commitment to integrated, team-based care.

  • Ability and willingness to adapt to a value-based care model.

  • Exceptional patient service and relationship-building skills.

  • Basic computer proficiency.

  • Active BLS certification.

  • Current fingerprint clearance card.

  • Active DEA license.
