What You’ll Do:
In this role, you’ll be a core member of an integrated clinical team, providing direct patient care and contributing to ongoing quality and population-health initiatives. You’ll help shape a patient experience rooted in accessibility, respect, and clinical excellence. Responsibilities include:

Providing comprehensive evaluation and treatment for patients following established standards of care.

Monitoring and improving patient outcomes through chart review, data insights, and adherence to care pathways.

Coordinating services across the care team and referring to additional resources when appropriate.

Participating in daily huddles and serving as a trusted source of medical expertise for the clinical staff.

Fostering a professional, collaborative, and supportive practice environment.

Completing documentation efficiently and accurately.

Supporting organizational policies, protocols, and quality initiatives.

Engaging with cross-functional teams regarding patient care quality, outcomes, procedures, and operational needs.

Exploring opportunities for growth and enhancement of clinical services.

Maintaining all required licensure, certifications, and compliance standards.

Building positive relationships with community partners and healthcare collaborators.

Working onsite 100% of the time, with occasional movement between clinics as needed.

Performing additional duties as assigned.
What We’re Looking For:

Current and unrestricted Arizona medical license.

Completion of an accredited MD or DO program.

Excellent interpersonal, verbal, and written communication skills.

A collaborative mindset and enthusiasm for team-based, integrated care.

Adaptability and a desire to thrive in a value-based care environment.

Strong patient-focused service orientation.

Basic computer competency.
-
Active BLS certification.
-
Active fingerprint clearance card.
-
Active DEA license.
