Responsibilities and Duties
- Collaborate with up to two advanced practice providers
- Manage medications and treatment plans for patients
- Educate patients about home treatment and preventive care
- Refer patients to specialists as needed
- Ensure continuity of care between visits
- Complete charting and patient documentation in the EMR
- Participate in peer reviews
- Work in conjunction with the Chief Medical Officer and medical team
- Provide care to a culturally and economically diverse patient population
Additional Information
- Must hold an active Indiana medical license
- Board certification in Family Medicine required
- Comfortable providing care for all age groups
- Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR) certifications required
- Experience with EMR systems preferred
