- Other details: The facility is looking for a provider to cover on-site clinical hours only. There are an average of 16 patients per shift. Candidate CVs must include the month and date for all work experience, with any employment gaps fully accounted for. The role includes some chronic pain management, requires chart closure within 72 hours, and follows a schedule of Monday–Friday, 8–5, with a minimum of 18 patients per day across a mix of pediatric and adult populations (FP-only candidates will be considered). Preference is given to providers who can see infants, though ages 12+ is required. Decisions are typically made within 7–14 days. MDs and DOs are accepted, and BE candidates will be considered if they are truly BE within 5 years of residency completion. An active DEA is required before the start date. Credentialing typically takes 30–90 days, with six weeks being common if all documents are submitted promptly. Required immunizations prior to the start date include the 2022–2023 flu vaccine, TB status or X-ray results within 90 days of the start date, positive MMR titer or completed series, positive Hep B titer or completed series, positive varicella zoster titer or completed series, and Tdap within the last 10 years. A 30-day notice is required for ending any assignment.
