Locums Primary Care Physician

We are seeking a compassionate and mission-driven Locums Primary Care Physician to join our interdisciplinary team in Fresno, CA as part of our Program of All-Inclusive Care for the Elderly (PACE). This unique, value-based care model enables providers to build deep, long-term relationships with a smaller panel of patients—focusing on quality, not quantity.

As a Primary Care Physician within our holistic, team-based model, you'll experience the rewarding impact of working with frail, underserved older adults in a setting that prioritizes comprehensive, patient-centered care over volume-based medicine.

Compensation: $140-$160 Per/hour+ Travel reimbursement.

Position Highlights:

• Provide primary care services to a panel of geriatric patients enrolled in the PACE program
• Collaborate closely with an interdisciplinary team including nurses, therapists, social workers, and other specialists
• Deliver patient-centered, preventive, and chronic care in an outpatient clinic setting
• Daily patient volume is 8 in the clinic and 4 in field per day
• Participate in care planning, case management, and care coordination for complex patients
• Enjoy a manageable patient load and extended time with each participant

Qualifications:

• MD or DO degree required
• Board Certification in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics
• Active California medical license and DEA (or ability to obtain)
• Must be eligible for enrollment with Medicare and Medi-Cal
• Prior experience in geriatrics or working with frail, medically complex adults preferred
