SIGN ON / RELOCATION BONUS NEGOTIABLE
Job Type: Full-time or Part-time
Setting: Skilled Nursing Facilities, Assisted Living Centers, Memory Care
Location: Warwick, Rhode Island
About the Role
We are seeking a dedicated Primary Care Physician (Internal Medicine, Family Medicine, or Geriatrics) to provide high-quality patient care across Skilled Nursing Facilities, Assisted Living Facilities, and Post-Acute Care settings. This is a unique opportunity to join a structured care model that delivers primary care directly to patients in their living environment.
The incoming physician will enjoy flexible scheduling, independence, and the opportunity to take on Medical Directorships across multiple facilities.
Key Responsibilities
Provide face-to-face primary care services within skilled nursing and assisted living settings.
Manage acute, chronic, and post-acute medical conditions among geriatric and rehab patients.
Conduct exams, manage medications, order labs/imaging, and coordinate care.
Collaborate with nurses, NPs, and PAs to ensure seamless patient care.
Oversee 5-8 assigned facilities, visiting each twice per week.
Serve as Medical Director for assigned facilities (optional additional income).
Participate in limited rotation-based call coverage (with option to sell call shifts).
Schedule & Call Details
Monday to Friday - Round-and-Go model
No shift work; visit your facilities and complete documentation
Overnight, weekend, and holiday coverage handled by outsourced tele-service
2-5 buildings without tele-coverage rotate call once a month
Patient Volume
Average: 15-20 patients per day
Benefits (W-2)
3 weeks vacation
2 weeks sick time
6 paid holidays

Qualifications
MD or DO with training in Family Medicine, Internal Medicine, or Geriatrics
Active or eligibility for Rhode Island medical license
DEA license
Experience in skilled nursing or post-acute geriatric care preferred
New graduates considered if exposure to geriatrics/Post-Acute Care
Must be comfortable working collaboratively and independently
