Clinical Role:

100% outpatient family medicine with opportunity for dedicated pediatrics time if desired

See ~20 patients per day

Team-based medical home model with on-site dental, optometry, behavioral health, lactation support, parent educators, diabetic nurse educator, and MAT program

Due to patient population, must be bilingual in Spanish

Compensation & Benefits:

Salary: $270,400 – $304,337.58 annually (DOE & FTE)

Sign-on/Retention Bonus: Up to $30,000 (FTE prorated)

50% paid on first paycheck, 25% after year 1, 25% after year 2

Relocation assistance: Up to $10,000
