- Schedule = minimum see 18 PPD
- A mix of peds/adults
- Includes some Chronic Pain Management
- 1 RN, 2 MAs on staff
- Requirements: BC, BE FM, CA license, Active DEA, BLS
- Epic EMR
- Credentialing takes 30-90 days
