Start Date: February 23, 2026
Pay Rate: $185/hr | 1099 | No Call
Schedule: Monday–Friday | 8:00 AM–5:00 PM | Minimum 18 patients/day

- Outpatient only – no call responsibilities
- Chronic Pain Management – light volume, integrated into primary care
- Charting – EMR with 72-hour chart closure requirement
- Patient Mix – Peds & Adults (12+ required; infant care preferred but not mandatory)
- Credentialing – 30–90 days (avg. 6 weeks with timely submission)
Qualifications
- Active DEA – must be in place prior to start
- BLS Certification
- Immunizations (required prior to start):
- 2022–2023 Flu Vaccine
- TB Status/X-Ray (within 90 days of start)
- Positive MMR, Hep B, and Varicella Zoster titers or complete immunization series
- Tdap (within the last 10 years)
