Job Function:

• Organization making investment in more community health provide care to Medicaid patients at Article 28 facility on the hospital campus + have private practice community locations surrounding the hospital.
• Expect to spend 40% of time at hospital clinic + 60% at one of the outside locations.
• Primarily adult care but if family trained would have the ability to see pediatrics/adolescents.
• Expect patient load up to 16 per day, likely build up to 40 min slots for new patients and 20 min follow ups.
• If have MPH or other training, could have dedicated time for community events, boards and program development.
• Athena EMR and practice qualifies for PSLF.

• BE or BC in Family Medicine or Internal Medicine.
• Active NY State License Required.
• Current ACLS or BLS Required.
