Requirements:
- Active CA License
- BC/BE in FM (within 5 yrs post residency)
- BLS
- DEA
- CVs must have month & date on work experience w/ all gaps in employment accounted for
- Immunizations such as Flu Vaccine, TB Status/X-Ray Results etc
