Nurse Practitioner Advanced Primary Care (Field-Based) | $145K+ Potential | Las Vegas, NV
Compensation: $116,000$145,000 base salary + annual bonus eligibility
Schedule: MondayFriday, no nights, weekends, or call
Were seeking a rockstar Nurse Practitioner (NP) to join a leading, patient-focused primary care group delivering advanced care to patients across the Las Vegas and Henderson areas. This role is ideal for an NP who thrives on autonomy, collaboration, and meaningful patient impact providing care where its needed most.
Youll visit patients in Assisted Living, Memory Care, Group Homes, and Hospice settings (no personal home visits), managing a census of roughly 90 patients. Youll collaborate closely with a Primary Care Physician, ensuring comprehensive, compassionate, and coordinated care.

Partner with an assigned PCP and interdisciplinary care team

Responsibilities
Conduct assessments, develop and manage care plans, and coordinate follow-up care
Provide chronic disease management, preventive services, and palliative care support
Maintain professional communication with patients, families, and facility staff
Navigate multiple facility environments confidently while fostering positive relationships
Document all encounters accurately and efficiently in the EMR
Qualifications
Masters Degree in Nursing (MSN)
Current, unrestricted RN and NP licenses in Nevada
Minimum 1 year of NP experience (Primary Care, Geriatrics, or Palliative Care preferred)
Excellent communication, problem-solving, and patient-relationship skills
Ability to work independently while collaborating with a broader care team
