Compensation: $232K$290K base + annual bonus Schedule: MondayFriday, no nights or weekends

Role Highlights
100% post-acute rounding in partnered SNFs
Focus on prevention, chronic-condition management & readmission reduction
Collaborate with NPs, RNs & facility staff in a multidisciplinary model
Participate in value-based care initiatives and quality metrics

Requirements
MD or DO with active CA license
BC/BE in Internal Medicine, Family Medicine, or Geriatrics
Experience in post-acute or long-term care preferred
Skilled in goals-of-care conversations & documentation standards
