#### Position Highlights:
Setting: Skilled Nursing Facilities (SNFs)
Schedule: Monday–Friday (flexible rounding schedule)
Responsibilities Include:
- Manage chronic disease and acute medical conditions
- Develop and implement individualized care plans
- Collaborate with interdisciplinary teams and facility leadership
- Provide mentorship and oversight to Nurse Practitioners
- Conduct clinical case reviews and support quality improvement initiatives
- Partner with leadership on care coordination and patient management strategies

#### Candidate Qualifications:
Board Certified in Family Medicine (FM), Internal Medicine (IM), or Emergency Medicine (EM)
Active and unrestricted Delaware Medical License (or application in process)
Active DEA and prescriptive authority (or eligibility)
Minimum 1 year of primary care experience; post-acute care experience preferred
Strong communication and leadership skills

Annual Compensation: $210,529 – $270,529
