- Seeking a board certified or truly board eligible family medicine physician (MD or DO) dedicated to join our team in providing full-scope, evidence-based primary care to a diverse patient panel across the lifespan.
- The ideal candidate will deliver comprehensive care that includes preventive services, chronic disease management, and coordination of specialty referrals, all within a collaborative, mission-driven environment focused on improving community health outcomes.
- Full-time: Mon-Fri (9a-5p)
- Key responsibilities: Diagnose, treat, and manage acute and chronic medical conditions, Provide preventive care, including immunizations, screenings, and health maintenance, order, interpret, and follow up on labs, imaging, and diagnostic studies,Refer to specialists and coordinate continuity of care,Allocate approximately 20% of time to administrative duties (e.g., chart review, team meetings).
- Qualifications: MD or DO from an accredited medical school, ?Completion of an accredited residency in Family Medicine or related specialty, Current, unrestricted California medical license and DEA registration,BLS and ACLS certification,?Proficiency with electronic health records (EHR),2–5 years post-residency clinical experience preferred.
- Multilingual skills Required: (Korean, Persian, Russian, or Mandarin).
- Must be very driven and experienced in substance abuse disorders
- Experience in community health or underserved populations.
- Familiarity with chronic care models and population health management.
