Market Medical Director | Value-Based Care Leadership | Enterprise Role with Clinical Component

A unique opportunity for a mission-driven physician leader to shape the future of value-based senior care. This role combines executive-level leadership with a limited clinical practice, ensuring alignment between strategy and patient care delivery.

Position Highlights:

• Split role: 80–90% leadership / 10–20% clinical practice
• Lead multiple primary care centers supported by comprehensive, multidisciplinary teams
• Oversee programs in chronic disease management, prevention, care transitions, and disparities reduction
• Mentor and coach medical directors and local providers across the market
• Collaborate with operations, analytics, and technology teams to advance population health strategies
• Deploy care pathways and risk stratification tools tailored to a fully capitated, value-based model
• Ensure clinical quality, patient experience, and cost optimization

Compensation & Benefits:

• Generous base salary:
• First Year Earnings (Salary plus bonuses/incentives) = $385k - $399k
• Sign-on bonus
• 25 PTO days, 5 CME days, 9 holidays (2 floating)

Ideal Candidate:

• MD/DO, board-certified in Internal Medicine, Family Medicine, Geriatrics, or related specialty
• 8–10 years of clinical practice, with at least 5 years of proven leadership experience
• Background in value-based care delivery and population health management
• Strong executive presence with excellent communication and mentoring skills
• Demonstrated success leading enterprise-level programs or care model innovations
• Commitment to health equity and care transformation
• Ability to travel regularly across assigned market centers
