Home-Based Primary Care Opportunity | $290k+ | Virgina Beach

Opportunity Highlights:

• Monday–Friday schedule: 8:00 AM to 4:30 PM, no weekends
• Weekly high-risk huddles with your advanced practice team
• 4 hours of dedicated collaboration time per NP per week, including ride-alongs for patient visits
• Remaining weekdays focused on direct patient care — 7 patient contact hours daily with Medical Assistant driver support (chart in transit, no after-hours admin work)
• Manage a patient panel of 150–170 individuals while leading 4 Nurse Practitioners
• Full-time providers carry an average panel of 275 patients
• Work alongside social workers, care managers, navigators, and community health partners
• Shared call, rotated weekly across all providers in the state (triage team takes first call)
• Dyad leadership partnership with your Market Manager

Compensation & Benefits:

• Base Salary: $260,000–$290,000, with $290K targeted for these roles
• Annual Bonus: $40,000–$60,000, based on productivity, quality metrics, and team performance
• Retention Bonus: $75,000, paid over three years
