Position Overview:
* Provide comprehensive primary care to a diverse patient population
* $275,000.00- $280,000.00
* Diagnose and manage acute and chronic conditions
* Coordinate care with specialists and interdisciplinary teams
* Promote wellness and preventive care
* Maintain accurate documentation in EMR
* Participate in quality improvement and team-based initiatives
