Job description
We are a rural, community health center serving approximately 13,000 patients/42,000 visits annually. The health center operates four brick-and-mortar health centers; five rural school-based health centers, and a mobile health unit that provides outreach to the patients in our community with primary care services. We are an integrated practice that centers the patient around primary care and integrates behavioral health, pharmacy, chronic care management, substance use treatment, and other ancillary services to support the whole person. We support our providers by allowing them to tailor their practice to their area of interest. Primary care physicians provide wellness, chronic, and urgent care and may offer procedures.

This is a health center that focuses on removing barriers to care and giving our providers the opportunity to tailor their practice to areas of interest.
