Position Overview:

A mission-driven, community-focused healthcare organization is seeking a compassionate and skilled Family Medicine or Internal Medicine Physician (MD or DO) to join a dynamic team in South Bend, Indiana. This role offers an opportunity to make a meaningful impact by providing high-quality, affordable healthcare to diverse and underserved populations in an outpatient setting.

Whether you're a seasoned provider or a recent graduate, this is your chance to be part of a collaborative, patient-centered care model with integrated services across medical, dental, and behavioral health disciplines.

Key Responsibilities:

  • Provide comprehensive outpatient primary care to a diverse patient population

  • Collaborate with a multidisciplinary team including physicians, nurse practitioners, dentists, and behavioral health specialists

  • Maintain accurate documentation using eClinicalWorks (eCW) EMR

  • Participate in quality improvement initiatives and team-based care

  • Support and embody the mission of providing compassionate, affordable care to all

Work Schedule:

  • Monday to Friday daytime hours (no evenings)

  • Four-day workweek optional

  • No weekends required

  • Major holidays off

  • Dedicated administrative time included in weekly schedule

Qualifications:

  • M.D. or D.O. from an accredited medical school

  • Completion of an accredited residency in Family Medicine or Internal Medicine

  • Board-Certified or Board-Eligible

  • Eligible for medical licensure in the state of Indiana

  • Active DEA license (or ability to obtain prior to start)

  • Strong commitment to community health and serving underserved populations

  • Prior experience or interest in working within FQHC, CHC, or PCMH settings is preferred
