JOB OVERVIEW
• Job Title: Primary Care Physician – Family Medicine / Internal Medicine with Addiction Focus
• Job Type: Locum Tenens
• Location: Stamford & Cheshire, CT
• Service Setting: Outpatient – Primary Care
• Coverage Type: Clinical Only
• Coverage Period: Sept 1, 2025 – Mar 31, 2026 (6–9 months)
• Clinical Shift Schedule: Monday – Friday, 8:00 AM – 5:00 PM with 1 hour break
PATIENT INFORMATION
• Patient Demographics: Pediatrics (age 5+), Adults, Geriatrics
• Patient Volume/Census: 15–20 patients per day
• Phone Consults: Yes
• Case Mix: Family Medicine, Internal Medicine, Addiction Medicine
FACILITY INFORMATION
• EMR System: Epic
• Office Equipment Available: Standard outpatient setup
• Support Staff Available: MA and LPN
• Reason for Coverage: Supplemental

REQUIRED PROCEDURES
• Preventive primary care
• Management of chronic conditions
• Addiction medicine evaluations and follow-up
JOB REQUIREMENTS
• Licenses: Active Connecticut medical license
• Board: BC/BE in Family Medicine or Internal Medicine
• DEA: Active DEA Certification
• Certifications: BLS, ACLS preferred
• Experience: Minimum 2 years preferred in outpatient and/or addiction medicine
• Other Qualifications: Ability to see patients aged 5+; strong EMR proficiency
DUTIES & RESPONSIBILITIES
• Provide outpatient primary care and addiction treatment to a diverse patient population
• Conduct routine physical exams, chronic disease management, and preventive care
• Treat and manage patients with substance use disorders
• Collaborate with MA and LPN for clinical operations
• Maintain accurate records in EMR
• Utilize 4 hours of weekly administrative time for documentation, follow-up, and coordination of care
