Job description
We are seeking a compassionate and dedicated Primary Care Physician to join our care team at a dynamic, patient-centered medical center. The ideal candidate will be committed to delivering high-quality care, participating in team-based treatment planning, and ensuring a seamless patient experience.
Key Responsibilities:

Clinical Care: Evaluate and treat center patients in accordance with clinical standards of care and organizational protocols.

Care Coordination: Assist in managing patient services, including specialty referrals, hospital and SNF transitions, durable medical equipment, and home health care coordination.

Team Collaboration: Act as an integral member of the care team by actively participating in daily huddles and case discussions.

Leadership Support: Partner with the Lead Physician and Center Administrator to promote a professional, collaborative, and patient-first culture.

Documentation: Complete accurate and timely medical record documentation with support from a quality-based coder to ensure comprehensive care notes.

Compliance: Follow clinical policies, procedures, and protocols as outlined by Clinical Leadership.

Quality Review: Participate in quality-of-care reviews, outcome evaluations, and medical record audits with the Lead Physician.

Focused Patient Care: Dedicate the majority of your time to direct patient care, including face-to-face visits and related administrative responsibilities (e.g., charting, care planning).
Qualifications:

MD or DO degree from an accredited medical school

Board certification or eligibility in Internal Medicine or Family Medicine

Active, unrestricted medical license in [Insert State]
