Family medicine Physician-Rockland, ME (Open to New Graduates)
Shift: Full Time/Part Time (Monday through Friday, 8 a.m. to 5 p.m.)

Job Summary & Responsibilities:

The Primary Care Physician is a clinical leader responsible for delivering high-quality, patient- and family-centered care to a diverse and underserved population. Working collaboratively with a multidisciplinary team, the physician focuses on prevention, chronic disease management, and health promotion within a community health clinic setting.

Perform comprehensive assessments, including histories, physical exams, and diagnostic testing.

Diagnose and treat a broad range of acute and chronic conditions (e.g., diabetes, hypertension, asthma).

Provide preventive services such as immunizations, screenings, and lifestyle counseling.

Coordinate care and refer patients to specialists when necessary.

Participate in quality improvement and patient safety initiatives.

Collaborate with community partners to reduce health disparities and improve access to care.

Support health education and outreach programs.

Advocate for policies that enhance care for underserved populations.

Document care accurately and timely in the EHR.

Manage referrals, follow-ups, and contribute to care coordination.

Attend team meetings, case huddles, and CME activities.

Professional Standards

Adhere to all terms of the Provider Employment Agreement.

Maintain strict confidentiality regarding patient and organizational information in accordance with HIPAA and clinic policies.

Support a safe and respectful workplace for patients and team members.
Education, Licensure, and Experience:
-
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
-
Board certification in Primary Care specialties such as Internal Medicine, Family Medicine, or Pediatrics.
-
Possession of an unrestricted license to practice medicine in the state of Maine.
-
Valid and current U.S. Drug Enforcement Administration (DEA) license.
-
Minimum of three (3) years of clinical experience in a community health setting; experience in a Federally Qualified Health Center (FQHC) strongly preferred.
