PATIENT INFORMATION
• Patient Demographics: General primary care population
• Phone Consults: Yes
• Case Mix: Family Medicine or Internal Medicine – outpatient only
FACILITY INFORMATION
• EMR System: EPIC – fully integrated HIMSS Stage VII
• Office Equipment Available: Standard primary care equipment
• Specialty Backup Available: Collegial group of system physicians
• Support Staff Available: Nurse practitioners in outpatient settings
• Reason for Coverage: Growth and expansion of physician services
PRIVILEGES & COMPLIANCE
• Credentialing Timeline: 90–120 days post-offer

JOB REQUIREMENTS
• Licenses: Active Indiana license (or ability to obtain)
• Certifications: Board Certified or Board Eligible in Family Medicine or Internal Medicine
• Experience: Open to all experience levels
• Other Qualifications: Supportive system for mentorship and development; EPIC proficiency
DUTIES & RESPONSIBILITIES
• Provide comprehensive outpatient care for a diverse patient population
• Collaborate with other system physicians in a collegial and growth-oriented practice
• Utilize EPIC EMR system for documentation
• Contribute to the expansion of primary care services in a growing health system
