Job Responsibilities
• Diagnose and manage acute and chronic medical conditions.
• Collaborate with behavioral health, dental, and social services teams to provide integrated care.
• Participate in quality improvement initiatives and population health management.
• Maintain accurate and timely documentation in the electronic health record (EHR).
• Engage in patient education and preventive care strategies.
