We are seeking a Permanent Primary Care Physician in California.

Position Type: Full-Time
Specialty: Physician – Primary Care (Internal Medicine, Family Medicine, or Geriatrics)
Location: Fresno, California
Rate: Open & Negotiable
Shift: Monday–Friday, standard business hours; rotating night/weekend call
Start Date: ASAP
End Date: Ongoing

Position Details:
Patient Population: Geriatric (PACE participants, typically 55+)
Setting: Community-based outpatient, nursing homes, and hospitals
Responsibilities:

• Conduct in-person comprehensive assessments

• Participate in IDT meetings to develop and review individualized care plans

• Provide and oversee medical care in clinic, hospital, and nursing home settings

• Collaborate with other providers and support teams to ensure continuity of care

• Provide direction to Nurse Practitioners and team members

• Participate in rotating on-call coverage

Required Procedures: General outpatient/geriatric primary care procedures
EMR System: Not specified
Support Staff: Interdisciplinary Team (IDT) including Nurse Practitioners and care managers
Days Per Month: Full-time schedule
On-Call/Weekends: Rotating night and weekend call

Submission Requirements:

• Active California Physician License

• Board Certification in Internal Medicine, Family Medicine, or Geriatrics (preferred)

• Completion of ACGME-accredited residency

• Active DEA and state CDS certification

• Eligible for Medicare and Medi-Cal enrollment
