Position Details:
Patient Population: All ages – pediatrics through geriatrics
Setting: Outpatient primary care clinic
Responsibilities: Provide comprehensive primary care, chronic and acute illness management, exams, screenings, immunizations, lifestyle counseling
Required Procedures: General outpatient care
EMR System: Not specified
Support Staff: Advanced Practice Provider (APP), RN triage support
Days Per Month: Full-time (4- or 5-day work week)
On-Call/Weekends: Call once every 70 days; telehealth 2–3 hrs on weekends once every 3 months
Submission Requirements:

Board Certified in Family Medicine OR dual Board Certified in Internal Medicine and Pediatrics – REQUIRED

Active NY license or willing to obtain – REQUIRED

At least 1 year of primary care experience – REQUIRED

Clean malpractice and licensing history – REQUIRED (no exemptions)
