- TRULY Board Eligible (within 5 years of eligibility with plans to test) or Board Certified in Family Medicine (will not accept Internal Medicine) - REQUIRED
- OH license (or willing to obtain) - REQUIRED

- Malpractice and/or Background Documentation
- Date of agency's last communication with provider (note in "Additional Submission Details") - REQUIRED
- Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - REQUIRED
- Provider's preferred location and schedule option (note in "Additional Submission Details" field) - REQUIRED
- Clean malpractice and license - REQUIRE

Job Responsibilities
Weekend Requirements : Varies by location - max of 1:3 office (phone call only).
On Call Requirements : Varies by location - max of 1:3 office (phone call only).
Diverse spectrum of interesting and complex patients
Ability to choose patient mix and practice at top of license
Weekly onsite CME, and CME/grand rounds available through Mayo Clinic Care Network
24-hour in-house Hospitalist services
Option to teach medical students and advanced practice trainees
Full complement of medical and surgical specialties available for support
State-of-the-Art Surgical and Inpatient Facilities and Technology

Working Hours
2) Week On/Week Off (7am-7pm Mon-Thurs, 7am-5pm Fri, 4hr Sat)
3) Twelve 10hr Days/28-Day Block (Wed-Fri, 3 days on/2 off, Mon-Wed, 3 days on/6 off)
